NOTE: You must have a physical rabies vaccine certificate for any rabies vaccinations documented on the health certificate. These vaccination certificates will be required by the USDA at the time of health certificate endorsement.

Be sure to check with your airline to determine any requirements they may have. (Airlines may have separate and additional requirements from those of your destination country). Airlines have additional forms to complete for service animals and/or animals going into cargo hold (acclimation letter).
